TRAC Services for Families would like your opinion!

Tell us how TRAC is doing and if anything can be improved. Your personal information will be kept confidential and only the combined responses will be shared with the agency. All questions are optional, please answer all questions if possible.

This survey pertains to services by TRAC Services for Families only. These services include: trainings, certification and recertification, the family profile writing process, therapeutic services (outpatient and in-home services), and family casework actions (transportation, child/parent visits, home visits, crisis services, etc). As a reminder, TRAC has minimal-to-zero influence on decisions made by the court system and County Children Youth and Families Departments. These decisions (such as reunification, visit changes, court ordered services) are outside of our influence as a third party service provider and should not be considered when filling out this survey.
